Important forms
Below are links to important forms. The first is (1) Sport Physical, (2) Concussion release form and (3) Information sheet on SuddenCardiac Arrest , and . These three documents are 10 pages total. Please return ALL 5 pages of the (1) TSSAA Physical Form, the LAST page of the (2) TSSAA Student/Parent Concussion Form, and the LAST page of the (3) Sudden Cardiac Arrest Form to the coach. Physicals, Concussion form, and Sudden Cardiac Arrest forms are good for the school year in which the student receives one; on April 15th each year, a student may get a physical that will be good for the next school year. If a student gets a physical prior to April 15th (for example, in March), they will need to get a NEW physical for the upcoming school year. Please be aware of this TSSAA rule and schedule physicals after April 15th. Any questions, please contact the Athletic Trainer or Athletic Director.

With Tennessee passing a new concussion law on January 1, 2014, if a student athlete is diagnosed with a concussion by me, they will need to see their primary care physician and have a TSSAA concussion return to play form filled out.
In April 2015, Tennessee passed the Sudden Cardiac Arrest Prevention Act. This act is to help prevent sudden cardiac arrest or potentially discover underlying heart conditions in student-athletes that may not have been detected before due to an onset of symptoms from physical activity. If an athletic trainer or other medical professional pulls a student-athlete from participation due to symptoms of sudden cardiac arrest or underlying heart condition, the student will need to see a licensed health care professional to be cleared and to have the Sudden Cardiac Arrest Medical Release form completed.
